Treatments

Partial Cornea Transplant in India

Quick answer

A partial cornea transplant (lamellar keratoplasty) replaces only the damaged layer of the cornea, keeping healthy layers in place. DMEK and DSAEK replace the inner layer; DALK replaces the front layers. It suits Fuchs dystrophy, corneal swelling and keratoconus. Overseas patients often stay about one to two weeks.

Rather than cutting out a full-thickness button, the surgeon swaps just the diseased layer. For inner-layer disease, a thin donor sheet is inserted through a small cut and held against the cornea with an air bubble. For front-layer disease, the patient's own inner lining is kept, which lowers rejection risk.

These techniques need specialised training and good donor tissue preparation, and are offered by cornea specialists at larger eye centres in India. Vision often returns sooner than with a full transplant, and the eye is structurally stronger.

How this approach differs

Compared with full-thickness keratoplasty, partial transplants keep the patient's healthy layers, so rejection is less common and the eye is less fragile after injury. Endothelial grafts use a small incision with few or no stitches, giving faster visual recovery. DALK takes longer to perform and can need conversion to full thickness if the inner membrane tears. Availability depends on surgeon expertise.

Who needs partial cornea transplant

  • Fuchs endothelial dystrophy causing morning blur and glare
  • Corneal swelling after cataract surgery (bullous keratopathy)
  • Keratoconus with a healthy inner lining, suitable for DALK
  • Front-layer scars from infection or injury
  • Failed earlier endothelial graft

How it is done

  1. Specialised imaging checks which corneal layers are affected.
  2. For DMEK or DSAEK, the damaged inner layer is stripped through a small side cut.
  3. A thin donor graft is rolled, inserted and unfolded inside the eye.
  4. An air or gas bubble presses the graft into place.
  5. For DALK, the front layers are peeled away down to the inner membrane and replaced with donor tissue stitched in place.
  6. The patient lies face-up for some hours after endothelial grafts to keep the bubble positioned.

Before you travel to India

  • Send corneal thickness and endothelial cell count reports if available.
  • Inform the team about glaucoma, eye drops in use or previous eye surgery.
  • Plan to avoid flying until the gas bubble has cleared, as advised by the surgeon.
  • Bring an attendant to help during the face-up positioning period.

Recovery and flying home

  • Most procedures are day care.
  • Face-up positioning is needed for a short time after endothelial grafts.
  • Vision often improves within weeks after DMEK or DSAEK.
  • Flying is permitted once the bubble has absorbed and the graft is attached.
  • Regular follow-up continues locally, with drops tapered over months.

Risks

  • Graft detachment needing a repeat air injection
  • Raised eye pressure from the bubble
  • Graft rejection, though less frequent than with full-thickness grafts
  • Conversion to full-thickness transplant during DALK
  • Infection

Alternatives

  • Full-thickness (penetrating) keratoplasty
  • Scleral contact lenses, for keratoconus
  • Cross-linking, for early progressive keratoconus

Frequently asked questions

What is the difference between DMEK and DSAEK?

Both replace the inner layer of the cornea. DMEK uses an extremely thin graft, while DSAEK includes a little more tissue. Your surgeon chooses based on eye condition and experience.

Why must I lie on my back after surgery?

The air bubble floats upward and presses the graft into place. Lying face-up keeps it in the right position. Follow the exact timing your surgeon gives.

Can I fly with gas in my eye?

No. Gas expands at altitude and can raise eye pressure dangerously. Wait until your surgeon confirms the bubble has gone.

Does DALK work for keratoconus?

Yes, DALK is often used for keratoconus when the inner layer is healthy. Keeping that layer lowers rejection risk. Some cases still need a full-thickness graft.

How long should an overseas patient stay in India after a partial cornea transplant?

Many patients stay around one to two weeks so the graft and bubble can be checked. If a repeat air injection is needed, the stay extends. Follow-up can then continue at home.
