Sperm freezing in India

A man produces a sample by masturbation, often after two to five days without ejaculation. The laboratory checks count and movement, mixes the sample with a protective medium and freezes it in small straws or vials.

Frozen sperm can be stored for years. When needed, it is thawed and used for the treatment that matches its quality. Lower-quality samples are usually reserved for ICSI, where one sperm is injected into each egg.

How this approach differs

Freezing several samples spreads the risk, because some sperm lose movement after thawing. If a man has very few sperm, the lab may use special small-volume freezing techniques. For cancer patients, freezing is ideally done before the first treatment dose.

Who needs sperm freezing

• Men about to start chemotherapy, radiotherapy or surgery that may affect fertility
• Partners who cannot be present on the day of their wife's egg collection
• Men with falling sperm counts who want a backup
• Men having a vasectomy who want to keep options open
• Sperm retrieved surgically through TESA or micro-TESE, so it can be used in later cycles

How it is done

1. A semen analysis is done first to judge count, motility and shape.
2. Infection screening blood tests (HIV, hepatitis B and C, syphilis) are taken.
3. One or more samples are produced in a private room at the clinic.
4. The laboratory adds cryoprotectant and divides each sample into several straws.
5. Straws are cooled in stages and stored in liquid nitrogen.
6. A test thaw of one straw may be done to check how well sperm survive.

Before you travel to India

• A recent semen analysis report, if available
• Infection screening results or time in India to complete them
• For cancer patients, a letter from your oncologist confirming the treatment start date
• Plan for two to four days in the city to provide more than one sample
• Ask how long the clinic can store samples and how you will renew storage from abroad

Recovery and flying home

• There is no recovery period after giving a sample.
• You can travel the same day.
• Storage is limited to ten years under the ART Act, 2021.
• When you want to use the sperm, the clinic thaws the straws for your partner's treatment.

Risks

• Some sperm do not survive freezing and thawing
• A sample may be too low in quality for anything except ICSI
• Very rarely, storage tank failure
• Stress or difficulty producing a sample on demand; clinics can advise on options

Alternatives

• Testicular tissue freezing for boys before puberty, at specialised centres
• Surgical sperm retrieval at the time of IVF, without prior freezing
• Donor sperm from a registered bank, if your own sperm cannot be used
