Ovarian Cancer Surgery in India

Quick answer

Ovarian cancer surgery, called cytoreduction or debulking, aims to remove all visible cancer in the abdomen and pelvis. It usually includes removing the ovaries, tubes, uterus and omentum, and sometimes parts of bowel or other organs. Hospital stays often last a week or more.

Ovarian cancer often spreads across the lining of the abdomen before it is found. The main surgical goal is to leave no visible tumour behind, because this strongly affects outcomes. Surgery may be done first (primary debulking) or after a few cycles of chemotherapy (interval debulking).

These operations can be long and extensive. They need gynaecologic oncologists with experience in upper abdominal surgery and intensive care support. Chemotherapy, and sometimes maintenance drugs such as PARP inhibitors, usually follows.

Approaches: standard, keyhole (laparoscopic)

Open surgery through a midline cut is the standard for advanced ovarian cancer because it allows inspection and removal of disease throughout the abdomen. Laparoscopy is used to assess whether full removal is possible, to stage early disease and occasionally for interval surgery in selected cases. The choice depends on how far the disease has spread.

Who needs ovarian cancer surgery

  • Newly diagnosed advanced ovarian, fallopian tube or primary peritoneal cancer
  • Early ovarian cancer needing staging surgery
  • Patients who have had chemotherapy and now need interval debulking
  • Selected recurrent ovarian cancer that can be fully removed
  • Suspicious ovarian masses needing removal and diagnosis

How it is done

  1. General anaesthesia is given with an epidural for pain control.
  2. A midline abdominal cut is made from below the breastbone to the pubic bone.
  3. The surgeon inspects all areas and assesses how much disease can be removed.
  4. The uterus, ovaries, tubes and omentum are removed.
  5. Tumour deposits on the bowel, diaphragm, spleen or abdominal lining are removed as needed.
  6. Lymph nodes may be removed, and bowel is rejoined or a temporary stoma created.
  7. The abdomen is closed and you are monitored closely after surgery.

Frequently asked questions

Why is complete removal of visible tumour important?

Studies show that leaving no visible disease is linked to better outcomes. Surgeons aim for this whenever possible. It sometimes requires removing parts of other organs.

Should I have chemotherapy before surgery?

If complete removal at first surgery is unlikely, chemotherapy may be given first. Interval surgery then follows. The team decides from scans and your fitness.

Will I need a stoma?

If part of the bowel is removed, a temporary stoma is sometimes needed. It is usually reversed later. Your surgeon will discuss this beforehand.

Should I have BRCA testing?

BRCA testing is recommended for many women with ovarian cancer. Results guide treatment, including maintenance drugs. Family members may also benefit from testing.

How long should an international patient stay?

Allow for a longer hospital stay and outpatient review afterwards. Chemotherapy may start in India or at home. Plan flexible travel dates.
