Laser Kidney Stone Treatment in India

Quick answer

Laser kidney stone treatment uses holmium or thulium laser energy, delivered through a thin scope, to break stones into dust or small fragments. It is used within ureteroscopy, RIRS and mini-PCNL. Many patients go home within a day, and international patients can often fly within a few days.

Here, laser is the tool used to break the stone, not a separate operation. A thin laser fibre passes through the working channel of a scope that reaches the stone. Laser energy can break even very hard stones.

Surgeons can either 'dust' the stone into fine particles that wash out, or fragment it into pieces removed with a tiny basket. Newer thulium fibre lasers and high-power holmium systems can make dusting faster. The approach suits most stone types.

How this approach differs

Compared with shock wave lithotripsy, which breaks stones from outside the body, the laser works directly on the stone under vision, so even hard stones can be broken reliably. Compared with older ultrasound or pneumatic devices used in PCNL, laser fibres are thin enough for flexible scopes and smaller access sheaths. The trade-off is that anaesthesia and a scope are needed, and a temporary stent is common. Holmium lasers are widely available in Indian urology centres, while thulium fibre lasers are found in fewer.

Who needs laser kidney stone treatment

  • Hard stones that do not break well with shock wave lithotripsy
  • Ureteric stones stuck in the tube between kidney and bladder
  • Small to medium kidney stones treated by RIRS
  • Patients on blood thinners who may not be able to stop them, in selected cases
  • Stones in people with obesity or unusual anatomy where shock wave therapy is less effective

How it is done

  1. The patient receives general or spinal anaesthesia.
  2. A scope is passed through the urethra and bladder into the ureter or kidney, or through a small back puncture.
  3. The laser fibre is advanced to touch the stone.
  4. Laser pulses dust or fragment the stone.
  5. Larger fragments are removed with a basket.
  6. A stent may be placed and the scope is withdrawn.

Before you travel to India

  • Non-contrast CT KUB scan with stone size and density
  • Urine culture to make sure there is no active infection
  • Kidney function tests
  • Previous procedure notes if stones were treated before

Recovery and flying home

  • Most patients go home the same day or the next morning
  • Mild burning and blood in urine for a few days is common
  • International patients can often fly within a few days if comfortable
  • Stent removal timing should be agreed before booking the return
  • A follow-up scan checks for leftover fragments

Risks

  • Leftover fragments that may pass later or need another session
  • Urine infection or fever
  • Stent-related discomfort
  • Injury to the ureter wall
  • Narrowing of the ureter later, which is uncommon

Alternatives

  • Shock wave lithotripsy, without a scope
  • PCNL with ultrasonic or pneumatic devices, for large stones
  • Medical expulsive therapy, for small ureteric stones

Frequently asked questions

Is laser treatment a type of surgery?

It is done during an endoscopic procedure under anaesthesia, but without cuts in most cases. The laser is passed through a scope that enters through the natural urinary passage.

What is the difference between holmium and thulium lasers?

Both break stones well. Thulium fibre lasers can create finer dust at some settings. Your urologist uses the equipment they know best.

Will the stone be removed completely?

The aim is to clear all fragments, but tiny pieces may remain. Many pass naturally. Larger leftover pieces may need another session.

Can very hard stones be treated with laser?

Yes, the laser can break stones of any hardness. This is one reason it is preferred for stones that resist shock waves.

How soon can I return home after laser treatment?

Many patients are fit to fly within a few days. Plan for stent removal before departure or at home. Carry your procedure summary.
