Laser Fistula Surgery in India
Quick answer
Laser fistula surgery (FiLaC) closes an anal fistula tunnel from inside using a laser fibre, without cutting the anal muscle. It suits selected fistulas, often high ones where muscle protection matters. Most patients go home the same or next day and can usually fly after about a week.

In laser closure, a thin radial laser fibre is passed through the fistula tunnel. As it is slowly pulled back, its energy shrinks and seals the lining of the tunnel. The track then closes over the following weeks.
The main aim is to avoid cutting the sphincter muscle, so bowel control is protected. Pain is usually mild and there is no large wound. However, healing rates can be lower than with traditional laying-open, and some patients need a repeat treatment.
How this approach differs
Conventional fistula surgery opens or cuts the track, which heals well but can affect the muscle and leaves a wound for weeks. Laser closure works from inside the tunnel, so there is no cut through the sphincter and recovery is quicker and less painful. The trade-off is a higher chance that the fistula does not close the first time. Candidates are usually chosen after MRI, and the method needs a surgeon trained in laser proctology.
Who needs laser fistula surgery
- High fistulas where cutting muscle would risk bowel control
- Patients who already have weak sphincter muscle
- Recurrent fistulas after earlier surgery, in selected cases
- Fistulas with a single clear track on MRI
- Patients who want a less painful option and are judged suitable
How it is done
- An MRI maps the track; a seton may be placed weeks before to let it mature.
- You receive spinal or general anaesthesia.
- The surgeon cleans the track with a brush or curette.
- A radial laser fibre is passed through the full length of the tunnel.
- The fibre is drawn back slowly while laser energy seals the track.
- The internal opening may be closed with a stitch or small flap.
Before you travel to India
- MRI fistulogram images and report
- Details of any seton currently in place and when it was inserted
- Records of previous fistula or abscess surgery
- Blood sugar control results if you have diabetes
Recovery and flying home
- Most patients go home the same day or after one night.
- There may be mild discharge for a few weeks as the track closes.
- Normal activities can often resume within a few days.
- A check-up before flying, often about a week after surgery, is usual.
- Healing is confirmed later, often by examination or repeat MRI at home.
Risks
- Fistula not healing, needing repeat laser or another operation
- Discharge or mild pain while the track closes
- Abscess forming if the track seals too early
- Bleeding (uncommon)
- Very small risk to bowel control compared with cutting methods
Alternatives
- Fistulotomy: lays open simple low fistulas with good healing
- LIFT procedure: ties off the track between muscle layers
- Advancement flap: covers the internal opening with healthy tissue
- Long-term seton: controls symptoms when repair is not advised

Frequently asked questions
Does laser fistula surgery work for every fistula?
No. It works best for single, well-defined tracks. Fistulas with many branches or active abscesses may need other methods first.
Why might a seton be placed before laser treatment?
A seton keeps the track draining and helps its lining become firm. This can make laser closure more effective. Some international patients have the seton placed at home before travelling.
Is there a risk of incontinence with laser closure?
The risk is very low because the sphincter muscle is not cut. That is one of the main reasons doctors choose this method. Your surgeon will explain your own risk.
What if the fistula does not heal after laser?
Laser can sometimes be repeated, or another sphincter-saving method can be used. A failed laser does not usually make later surgery harder. Your surgeon will discuss a plan.
How long should I stay in India for laser fistula surgery?
Many patients plan about one week. This covers tests, the procedure and an early check. Final healing is assessed later, often with your local doctor.
