Kasai Procedure in India

Quick answer

The Kasai procedure (hepatoportoenterostomy) treats biliary atresia, a condition where a baby's bile ducts are blocked or missing. A loop of intestine is joined to the liver so bile can drain. It works best done early in infancy. Families usually need a stay of a few weeks in India for surgery and recovery.

In biliary atresia, bile ducts outside the liver become inflamed and scarred in the first weeks of life, so bile cannot reach the intestine. Babies develop prolonged jaundice, pale stools and dark urine. Without treatment, bile damages the liver and leads to cirrhosis.

The Kasai procedure removes the scarred ducts and attaches a loop of small intestine directly to the liver where bile channels emerge. If bile flow is restored, many children keep their own liver for years. Others still need a liver transplant later. Speed of diagnosis and surgery matters greatly.

Approaches: standard, keyhole (laparoscopic)

The open Kasai procedure through a cut under the ribs is the established standard, with the largest body of outcome data. A laparoscopic Kasai uses small ports and a camera; it may mean smaller scars, but evidence on long-term bile drainage is mixed and it is offered at few centres. Surgeons usually recommend the open approach unless they have specific laparoscopic expertise.

Who needs kasai procedure

  • Babies with biliary atresia confirmed by tests
  • Jaundice lasting beyond the first couple of weeks of life with pale stools
  • Infants diagnosed early enough for liver drainage to work
  • Babies with jaundice, pale stools and an absent or abnormal gallbladder on ultrasound

How it is done

  1. Blood tests, ultrasound, a liver scan or biopsy and sometimes a cholangiogram confirm the diagnosis.
  2. Under general anaesthesia, the abdomen is opened through a cut under the ribs.
  3. The scarred bile duct remnant is removed.
  4. A loop of small intestine (Roux-en-Y loop) is prepared.
  5. The loop is joined to the surface of the liver at the bile channel area.
  6. The baby recovers in intensive care or a high-dependency unit.

Before you travel to India

  • Blood test results including bilirubin and liver enzymes
  • Ultrasound, scan and biopsy reports
  • Stool colour photographs
  • Urgent travel planning; delays reduce success
  • Guardian documents for consent

Recovery and flying home

  • Hospital stay is usually one to two weeks
  • Antibiotics and medicines to help bile flow are given
  • Jaundice clearance is monitored over weeks to months
  • Flying home is planned when feeding and liver tests are stable
  • Lifelong follow-up with a paediatric liver specialist
  • Liver transplant may be needed later

Risks

  • Cholangitis (infection of bile channels)
  • Failure of bile drainage
  • Progressive liver scarring
  • Bleeding or wound problems
  • Need for liver transplant

Alternatives

  • Primary liver transplant for babies diagnosed late or with advanced liver damage
  • Supportive care, which does not halt disease progression

Frequently asked questions

Why is timing so important?

Liver damage progresses quickly. Babies operated on early have a better chance of bile flow. Seek a specialist opinion urgently if your baby has jaundice and pale stools.

Will my child need a liver transplant?

Some children keep their own liver long term after a successful Kasai. Others need a transplant later in childhood. Regular follow-up guides this decision.

What is cholangitis?

It is an infection of the bile channels after surgery. Signs include fever, worsening jaundice and pale stools. It needs prompt antibiotics.

Can we travel quickly to India for a Kasai procedure?

Yes, share reports early so the team can review urgently. Medical visas can be expedited for urgent cases. Do not delay travel for non-medical reasons.

What feeding is needed after surgery?

Babies often need special formulas and vitamin supplements to support growth. A dietitian will guide you.
