Bentall Procedure in India
Quick answer
The Bentall procedure replaces the aortic valve, aortic root and the first part of the aorta with a single tube graft that contains a valve. The coronary arteries are then reattached to the graft. It is for an enlarged aortic root with valve disease, often in Marfan syndrome. Expect a few weeks in India.

At a glance
The aortic root is the section of aorta where the valve sits and where the coronary arteries arise. When the root widens, the valve leaflets can pull apart and leak, and the risk of tearing (dissection) rises.
In the Bentall operation, the surgeon removes the diseased root and valve and replaces them with a composite graft. Buttons of tissue containing the coronary artery openings are sewn into the graft. When the valve leaflets are healthy, a valve-sparing root replacement may be considered instead.
Who needs bentall procedure
- Aortic root aneurysm with a diseased or leaking aortic valve
- Marfan syndrome or other inherited aortic conditions with root enlargement
- Bicuspid aortic valve with root dilatation
- Type A aortic dissection involving the root
- Infection destroying the valve and root
How it is done
- General anaesthesia is given and the breastbone opened.
- The heart-lung machine is connected and the heart stopped.
- The enlarged root and aortic valve are removed.
- The coronary artery openings are cut out as small buttons of tissue.
- A composite valve-graft is sewn to the heart, and the coronary buttons are sewn into it.
- The graft is joined to the remaining aorta, and the heart is restarted and checked.
Before you travel to India
- CT angiogram or MRI of the aorta with images
- Echocardiogram with valve and root measurements
- Coronary angiogram or CT coronary angiogram
- Genetic test results or family history of aortic disease
- Dental clearance and blood tests
Recovery and flying home
- An intensive care stay of a few days is common.
- Ward recovery often takes about a week or more.
- Mechanical valve recipients need INR stabilisation before flying.
- Blood pressure control is important to protect the rest of the aorta.
- Lifelong imaging of the remaining aorta is advised.
Risks
- Bleeding from the many suture lines
- Problems with the coronary artery connections
- Stroke
- Heart rhythm problems or heart block
- Blood thinner complications with a mechanical valve
Alternatives
- Valve-sparing aortic root replacement (David or Yacoub procedures) when leaflets are healthy
- Separate valve replacement and ascending aorta graft when the root is normal
- Surveillance and blood pressure control for smaller roots

Frequently asked questions
What is the difference between Bentall and valve-sparing surgery?
Bentall replaces the valve along with the root. Valve-sparing surgery keeps your own valve and replaces only the root. Whether your valve can be kept depends on the leaflets.
Why is Marfan syndrome linked to this surgery?
Marfan syndrome weakens connective tissue, so the aortic root tends to enlarge. Early root replacement helps prevent dissection. Lifelong monitoring of the aorta is needed.
Is the Bentall procedure an emergency operation?
It is usually planned when scans show enlargement. It can be done as an emergency for aortic dissection. Planned surgery generally carries lower risk.
What should overseas patients send before a Bentall operation?
Send aortic CT or MRI images, echo reports and any genetic test results. These help the surgeon plan the type of graft and valve. Coronary imaging is often requested too.
Will I need blood thinners?
With a mechanical valve, lifelong warfarin is needed. Tissue-valve grafts may need only short-term treatment. Your surgeon will explain the options.
