ACL Reconstruction in India

Quick answer

ACL reconstruction rebuilds a torn anterior cruciate ligament in the knee using a tendon graft, usually through keyhole surgery. It is for active people whose knee gives way after an ACL tear. Most international patients stay in India about one to two weeks, then continue rehabilitation for several months at home.

The ACL is a key ligament that stops the shin bone sliding forward. It often tears during sports involving twisting or sudden stops. A torn ACL does not heal on its own, and a knee that keeps giving way can damage the cartilage and meniscus.

In reconstruction, a graft from the hamstring, patellar tendon or quadriceps tendon is passed through tunnels in the thigh and shin bones and fixed with screws or buttons. The graft becomes the new ligament over months. Return to sport is a gradual process guided by strength and stability tests.

Who needs acl reconstruction

  • Complete ACL tear with recurring knee instability
  • Athletes wishing to return to pivoting or contact sports
  • ACL tear combined with meniscus or other ligament injuries
  • Younger, active patients with a physically demanding job
  • Failure of physiotherapy to stabilise the knee

How it is done

  1. MRI confirms the tear and any meniscus or cartilage damage.
  2. Under anaesthesia, the knee is examined and arthroscopy is performed through small cuts.
  3. A tendon graft is harvested and prepared.
  4. Tunnels are drilled in the thigh and shin bones at the ligament's original attachment points.
  5. The graft is passed through and fixed with screws, buttons or both.
  6. Meniscus tears are repaired or trimmed in the same operation.

Before you travel to India

  • Send the knee MRI and any X-rays.
  • Begin 'prehab' exercises to reduce swelling and regain full straightening before surgery.
  • Tell the team about your sport and goals.
  • Plan for crutches and an aisle seat on the flight home.

Recovery and flying home

  • Most patients go home the same day or after one night.
  • Crutches are used for a short period.
  • Flying is generally possible after about a week to ten days, with clot precautions.
  • Return to running is usually several months later.
  • Return to pivoting sport often takes many months and requires passing functional tests.

Risks

  • Graft re-tear
  • Knee stiffness
  • Pain at the graft harvest site
  • Infection or blood clots
  • Later arthritis in the knee

Alternatives

  • Structured physiotherapy without surgery, for less active people
  • Activity modification and bracing
  • ACL repair (rather than reconstruction) in selected acute tears

Frequently asked questions

Which graft is best for ACL reconstruction?

Hamstring, patellar and quadriceps tendon grafts all work well. Each has small trade-offs. Your surgeon chooses based on your sport and knee.

When can I return to football or other sports?

Return to pivoting sport usually takes many months. You must pass strength and hop tests first. Rushing raises re-tear risk.

Do I need surgery for every ACL tear?

No. Some people with low activity levels do well with physiotherapy alone. Ongoing instability usually means surgery is advised.

Can a meniscus tear be fixed at the same time?

Yes, meniscus repair or trimming is often done during the same arthroscopy. Repair may slow early rehab a little. It helps protect the knee.

How does an international patient handle months of rehab?

The early phase is done in India, then a local physiotherapist continues. You receive a written protocol. Video reviews can track progress.
