Conditions

Micropenis

Quick answer

Micropenis is a penis that is normally formed but much shorter than expected for age, measured when gently stretched. It usually stems from low male hormone levels or poor hormone response before birth. Early hormone treatment in infancy or childhood often helps growth, and surgery is considered only for selected adults.

The diagnosis is made by a stretched length measurement compared with charts for the child's age. The urethra opens at the tip, and the shape is normal. A penis hidden by a fat pad or skin (buried or concealed penis) may look small but is not a true micropenis.

Most cases come from problems with the hormone signals from the brain's pituitary gland, the testes, or how the body responds to testosterone. Some children have other hormone deficiencies, so a paediatric endocrinologist (hormone specialist) and paediatric urologist usually work together.

Symptoms

  • Stretched penile length well below the normal range for age
  • Small or undescended testes
  • Small scrotum
  • Low blood sugar or prolonged jaundice in a newborn, suggesting pituitary problems
  • Delayed or absent puberty in older boys

Causes and risk factors

  • Hypogonadotropic hypogonadism (low hormone signals from the brain)
  • Testes that make too little testosterone
  • Partial androgen insensitivity (reduced response to testosterone)
  • Genetic syndromes such as Kallmann or Klinefelter syndrome
  • No cause found in some children (idiopathic)

How it is diagnosed

  • Stretched penile length measured with a rigid ruler
  • Blood tests for testosterone, LH, FSH and other pituitary hormones
  • Chromosome test (karyotype)
  • hCG stimulation test of testicular function
  • Pelvic ultrasound or brain MRI in selected cases

When a procedure is needed

Surgery is not the first treatment. Short courses of testosterone or other hormones are tried in infancy or childhood. Penile reconstruction or lengthening may be discussed for adults with persistent concern after hormone options, with honest counselling about limits and risks.

Frequently asked questions

Is a buried penis the same as micropenis?

No. In a buried penis, the shaft is normal but hidden by fat or skin. It often improves with weight loss or a minor operation. A micropenis has a truly short shaft.

When is hormone treatment given?

It is often given in infancy or early childhood, and sometimes around puberty. Short courses of testosterone are common. The specialist chooses the timing after blood tests.

Will my son be able to have children?

It depends on the cause. Many men with micropenis have normal sexual function and some are fertile. Hormone treatment at puberty can support fertility in some conditions.

What should we bring to the consultation?

Bring all hormone and chromosome results, growth charts and any scan reports. A record of earlier treatment helps. Older boys may want time to speak privately with the doctor.

Can follow-up continue in our home country?

Yes. Growth checks and blood tests can be done locally. Results can be shared with the specialist in India. Video reviews help plan the next step.
